DESCRIPTION OF OCCURRENCE:
On 08/04/17 a male patient (A), in his 50s, and another male patient (B), in his 30s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the physician and the police department. Patient (A) followed patient (B) into patient (B's) room and closed the door behind them. Staff immediately sounded the duress alarm for additional assistance. When staff arrived at patient (B's) room, they observed the two residents fighting. Staff and facility police officers entered the room and were able to escort patient (A) to the seclusion room where he was placed in locked door seclusion. Both residents were assessed and patient (A) was noted to have a small abrasion to his left ring finger. Patient B was assessed and noted to have an abrasion to his right arm and a small abrasion on his left knee. Both patients had a mental health diagnosis with a history of behavior problems which had been care planned. Patient (A) was moved to a different wing away from patient (B). Patient (A) was placed on assault II precautions.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
