DESCRIPTION OF OCCURRENCE:
On 7/25/17 male patient (A) assaulted male patient (B). Both patients were in their early teens.
The facility conducted an internal investigation and notified the police. Staff reported patient (A) was agitated about the activity program when patient (B) began antagonizing and laughing at patient (A) for being upset. Patient (A) responded by going after patient (B) and punching him on the face. Patient (A) then placed patient (B) in a choke hold and attempted to pull him away from the table. Staff intervened and placed patient (A) in a manual hold before escorting him to an evaluation area. Patient (B) complained of pain initially which later resolved. Staff noted redness to patient (B)'s cheek/jaw area. Patient (B) declined any treatments and reported he was not fearful. Staff assessed patient (A) and there were no reported injuries. Patient (B) was placed on assault I precautions and a peer restriction. Patient (A) was placed on assault II precautions and a peer restriction. In addition, both patients were placed in programming times opposite of one another.
