DESCRIPTION OF OCCURRENCE:
On 07/04/17 a male patient, in his 20s, and another male patient, in his 30s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the physician and the police department. Staff heard shouting coming from the male hallway and looked down the hallway and observed two patients fighting. Staff immediately ran down the hallway and physically contained patient (A). Staff cuffed patient (A) and escorted him to the seclusion room where he was placed in seclusion and restraint. Patient (B) was escorted to the day hall where he was assessed and noted to have a small bump to his left eye, his nose was slightly bleeding and he sustained a small cut in his upper gum inside his mouth. Both patients had mental health diagnoses and patient (A) had a history of behavior problems which had been care planned. The Medical Provider was notified and immediately wrote orders to place the assailant on assault II precautions.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
