DESCRIPTION OF OCCURRENCE:
On 06/28/17 male patient (A) began punching male patient (B). Both patients were in their teens.
The facility conducted an internal investigation and notified the police, physician and families/guardians. The patients were at Reactional Therapy group when patient (A) got off the couch and began hitting patient (B). Staff put patient (A) in a hold and lowered him to the ground. He was escorted to the seclusion room and placed in seclusion and restraint. He was assessed and had no injury. Patient (B) was assessed and had bruising and swelling to the back of his head and a complaint of pain. Vital signs and neurological checks were within normal limits. X-rays of the skull and orbits were ordered and showed no fractures. Orbits and sinuses appeared to be intact. Patient (A) later apologized to patient (B). Patient (B) was discharged. Patient (A) was placed on assault precautions and suicide precautions and removed from regular unit programming.
