DESCRIPTION OF OCCURRENCE:
On 06/25/17 a male patient, in his 20s, alleged a staff member injured him during a restraint episode.
The facility conducted an internal investigation and notified the police and physician. The patient and a staff member were involved in a verbal altercation that resulted in a staff member putting him in restraints. The patient admitted he resisted during the episode. Witness staff denied any inappropriate use of force by the accused staff member. The patient said he sustained a shoulder injury. He was assessed on 06/25/17 and was able to perform limited range of motion to his left arm and shoulder. He had an abrasion on his left shoulder and on his left hip with no bleeding or drainage. The patient refused Motrin but agreed to an x-ray. Shoulder x-rays were obtained on 06/26/17 and showed no acute fracture or dislocation. A referral for physical therapy was issued. The facility did not substantiate the allegation of abuse due to the patient's resistence and the statements of witnesses. The accused staff member was transferred to another unit where there would be no contact with this patient.
