DESCRIPTION OF OCCURRENCE:
On 06/21/17 a female patient (A), in her 50s, attempted to choke and cut the throat of another female patient (B), also in her 50s.
The facility conducted an internal investigation and notified the physician and the police department. Staff heard patient (B) yelling for help. Staff arrived at the patient's room and observed patient (A) with her hands on patient (B's) neck. Patient (A) was holding a tube of lotion and was attempting to cut her throat with the sharp, crimped edge of the lotion tube. Staff intervened and escorted patient (A) away from patient (B). Both patients had a mental health diagnosis with a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have several small scratches to the left side of her neck. Patient (A) received orders to be placed on one-to-one monitoring. Patient (A) stated patient (B) took some papers that belonged to her. Patient (A) was also placed on assault II precautions and moved to a more supervised unit. Patient (B) decided to press charges against patient (A).
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
