DESCRIPTION OF OCCURRENCE:
On 06/19/17 a male patient (A), in his 50s, struck another male patient (B), in his 30s.
The facility conducted an internal investigation and notified the physician and the police department. The two patients were arguing and yelling at each other. Staff observed patient (A) with his fist up yelling at patient (A) that he could not intimidate him. Patient (A) approached patient (B) and started punching patient (B) on the face. Staff separated the patients and escorted patient (A) to the seclusion room where he was placed in locked door seclusion. Patient (A) had just moved into patient (B's) room. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have redness and swelling to his left eye. Patient (A) was placed on assault II precautions and moved to another unit.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
