DESCRIPTION OF OCCURRENCE:
On 06/16/17 a male patient (A), in his 50s, and another male patient (B), in his 40s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the police department. Patient (A) came out of his wing and into the day hall and spit on patient (B's) face. Staff intervened and escorted patient (A) to locked door seclusion. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) stated he was fearful of patient (A). Patient (A) was maintained on assault II precautions and a peer restriction was ordered between the two patients. Staff were being vigilant about not allowing the two patients to be in the main day hall together. Patient (B) maintained assault I precautions.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
