DESCRIPTION OF OCCURRENCE:
On 06/16/17 a male patient (A), in his 50s, and another male patient (B), in his 40s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the police department. Staff heard patient (B) yelling and observed patient (A) and patient (B) in a physical altercation. Staff separated the patients and patient (A) was directed to his room. Patient (B) was assessed and noted to have a swollen lower lip with a laceration. Patient (A) was assessed and noted to have slight swelling on his nose and right cheek. Both residents had mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) was placed on assault II precautions and patient (B) was placed on assault I precautions. They were also placed on peer restriction from each other. Patient (A) was moved to a different wing of the unit and his medications were reviewed and revised.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
