DESCRIPTION OF OCCURRENCE:
On 06/16/17 a male patient (A), in his 20s, and another male patient (B), also in his 20s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the police department. Patient (A) had been very psychotic and responding to internal stimuli. Staff observed patient (A) and patient (B) fighting in the hallway. Staff intervened and separated the patients. Patient (B) stated the assault was unprovoked and they did not have any ongoing issues. Patient (B) was assessed and noted to have a cut on the inside of his mouth which was treated. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) continued on assault II precautions and his medication was being evaluated. Patient (B) was placed on assault I precautions for fighting back. Patient (B) was also moved to a different wing of the unit.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
