DESCRIPTION OF OCCURRENCE:
On 06/12/17 a male patient (A), in his 20s, struck another male patient (B), also in his 20s.
The facility conducted an internal investigation and notified the physician and the police department. Patient (B) was sitting in the hall when patient (A) came up to him and placed his fist in patient (B's) face. Patient (B) stood up and patient (A) began to hit patient (B). Staff intervened by placing patient (A) in a manual hold. Staff along with several officers arrived to assist and escorted patient (A) to the seclusion room. Patient (A) was not able to calm down and began to demonstrate increased agitation. Patient (A) was moved to another unit due to dangerous behaviors. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have a small abrasion to his right ring finger. Patient (A) was noted to have a small cut on his lip from being struck by patient (B). Patient (B) was discharged back to jail. Patient (A) was moved to different unit and placed on assault II precautions.
In accordance with Colorado Revised Statute 25-1-124, the Department conducted an off-site investigation of this reportable event and investigative findings submitted by the facility. This public summary is based on information provided by the facility to the Occurrence Section of the Department and is accurate and complete at this time. Prior to the next onsite investigation of the facility, this occurrence will be reviewed.
