DESCRIPTION OF OCCURRENCE:
On 06/07/17 male patient (A), in his 40s, hit male patient (B) on the head. Patient (B) was in his 30s.
The facility conducted an internal investigation and notified the police and physician. The patients were in the day hall and were arguing. Patient (A) then hit patient (B) on the head. Staff interviewed and escorted patient (A) to the seclusion room. He was placed in seclusion and restraints. Patient (B) was assessed and had a small scratch on his right ear that was not bleeding. His vital signs and neurological checks were within normal limits. He complained of a head ache and Tylenol was administered. Neurological checks every 2 hours for 24 hours were ordered. Patient (A) was transferred to another unit and placed on Assault II precautions. His medications were being reviewed.
