DESCRIPTION OF OCCURRENCE:
On 04/29/17, a male patient (A), in his 30s, and another male patient (B), in his 50s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the police department. The patients were standing in line waiting for medications. They began to exchange words and began pushing each other. Patient (A) began hitting patient (B) with his fists. Both patients fell to the floor and staff was able to contain patient (A). Staff walked patient (A) to the seclusion room and placed resident (A) in seclusion and restraint. Both patients had a mental health diagnosis and a history of behavior problems which had been care planned. The patients were assessed and patient (B) complained of tenderness to his head as he stated he bumped his head on a table he got under to get away from patient (A). Patient (A) was assessed and noted to have an abrasion on his left knee and on his left triceps. Patient (A) was placed on Assault ll precautions. Patient (B) reported feeling fearful of patient (A) so a peer restriction between the two patients was initiated.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review. The Department will review this occurrence prior to the next survey.
