DESCRIPTION OF OCCURRENCE:
On 04/15/17 a male patient (A), in his 20s, and another male patient (B), also in his 20s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the physician and the police department. Staff observed the two patients engaged in a physical altercation. Staff immediately responded and separated the patients. The patients were interviewed and the facility determined patient (B) initiated the altercation by asking patient (A) if he wanted to fight. Instead of answering patient (B), patient (A) punched patient (B) on the nose several times. Patient (B) was assessed and noted to have a bloody nose and x-rays were negative for fracture. Both patients on the unit had been verbally and physically aggressive but not with each other. The patients were in the High Security area of the facility due to their aggressive and unpredictable behavior. Both patients were on Assault ll precautions. Patient (B) was moved to another wing on the unit and both patients remained on Assault ll precautions. The psychiatric provider will continue to adjust the patients' medications in response to their behavior.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review. The Department will review this occurrence prior to the next survey.
