DESCRIPTION OF OCCURRENCE:
On 3/21/17 male patient (A), who was in his early teens, allegedly assaulted male patient (B), who was in his early teens.
The facility conducted an internal investigation and notified the physician and police. It was reported patient (A) and patient (B) were in the classroom when a staff member asked patient (A) to move and sit in a different seat. Patient (B) began to argue with patient (A) and then patient (A) proceeded to punch patient (B) in the face. Staff intervened and separated the patients away from one another. Staff escorted patient (A) to a quiet area to allow him to de-escalate without any further issues. Nursing assessed patient (B) who had a bloody nose which was cleansed. Staff continued to monitor him for any medical changes. Patient (B) reported he asked for staffs help on the computer when patient (A) told him to shut up, called him a name and then punched him. A peer restriction was implemented and patient (A) remained on assault II precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
