DESCRIPTION OF OCCURRENCE:
On 03/17/17 male patient (A), in his 50s, put his hands around male patient (B)'s neck and pushed him up against a wall. Patient (B) was in his 20s.
The facility conducted an internal investigation and notified the physician and police. Patient (A) put his hands around male patient (B)'s neck and pushed him up against a wall. Staff intervened immediately and were able to separate the patients. Patient (B) was assessed and vital signs and neurological checks were within normal limits. Patient (B) had slight redness to his neck but no other visible injury. He denied any pain. Patient (A) was placed on assault precautions and peer restriction and moved to another unit, away from patient (B). Patient (B) filed charges against patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings were based on the information provided by the facility at the time of the Department review.
