DESCRIPTION OF OCCURRENCE:
On 03/09/17 male patient (A), in his 40s, threatened male patient (B), who was in his 20s.
The facility conducted an internal investigation and notified police. Patient (B) reported to staff that patient (A) had threatened him. Staff then overheard patient (A) make these threats. Patient (A) went to his room and began hitting the window and becoming more aggressive. Staff asked him to walk to the seclusion room and he initially complied. He then said he would not go without a fight. He drew back his fist as if he was going to strike staff but did not follow through. He was placed in locked door seclusion. Patient (B) expressed fear of patient (A). Patient (A) was continued on assault precautions and wing observation. He was housed on a different wing than patient (B) and staff are monitoring him closely.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings were based on the information provided by the facility at the time of the Department review.
