DESCRIPTION OF OCCURRENCE:
On 03/10/17 female patient (A), in her 50s, hit female patient (B) who was in her 40s.
The facility conducted an internal investigation and notified the police and physician. Patient (A) came out of her room and hit patient (B) in the face several times. The assault was witnessed by a staff member. Patient (A), who was very paranoid and delusional, walked back to her room. Staff escorted patient (B) to her room. Patient (B) was assessed and had redness to her left cheek below the eye. She complained of pain and was given Ibuprofin. Patient (A) was continued on assault precautions and her medications were reviewed and adjusted. Patient (B) was moved to another unit. Both patients were placed on peer restriction.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings were based on the information provided by the facility at the time of the Department review.
