DESCRIPTION OF OCCURRENCE:
On 03/09/17 male patient (A), in his 20s, hit male patient (B) in the face. Patient (B) was in his 30s.
The facility conducted an internal investigation and notified the police. Patient (B) was getting a drink of water. He turned to go down the hall and patient (A) hit him in face with a closed fist. This happened without any warning or provocation. Staff placed patient (A) in a two man containment and escorted him to the seclusion room. He was put in locked door seclusion and placed on assault II precautions and peer restriction from patient (B). Patient (B) was assessed and had bleeding from his nose that quickly stopped. He had redness and slight swelling of his nose. An x-ray was taken and revealed a depressed nasal fracture of the tip of his nose. Ice packs were ordered for 3 days. Patient (B) refused Tylenol. Patient (A) was moved to another unit with a higher security level so patient (B) was no longer fearful.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings were based on the information provided by the facility at the time of the Department review.
