DESCRIPTION OF OCCURRENCE:
On 3/2/17 a male patient, in his 20s, eloped from the substance abuse program.
The facility conducted an internal investigation and notified the police and physician. This patient was transported to the medical clinical for a schedule appointment. Clinic staff reported he left the clinic without being seen by the physician. A search was conducted of the immediate area and campus. The patient was his own responsible party and had voluntarily admitted himself into this substance abuse program. Under a medical provider's guidance, the patient was discharged from the program as absent without authorization. The resident had a history of substance abuse and was at risk for returning to this lifestyle. Family reported they had not heard from the patient. At the time of this report, the patient has not been located. If he wished to return to the program, he would need to reapply.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
