DESCRIPTION OF OCCURRENCE:
On 2/25/17 male patient (A), in his 30s, physically assaulted male patient (B), who was in his 40s.
The facility conducted an internal investigation and notified the physician and police. Patient (A) approached patient (B) from behind. Patient (A) proceeded to kick patient (B) on the back of the head and then started punching him in the face repeatedly. Staff reported patient (B) did not hit back. The patients were separated and staff escorted patient (A) to the locked seclusion room. Nursing assessed patient (B) as he was complaining of pain and soreness to his face. Staff noted redness to patient (B)'s face above the cheek area. Staff continued to monitor his medical condition for any neurological changes. At the time of the incident, both patients were already on assault II precautions and restrictions. Patient (B) transferred to another unit and staff would continue monitoring both patients for re-direction as needed. The medical provider planned to review patient (A)'s mental health and behavioral medications. Patient (B) wished to press assault charges against patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
