DESCRIPTION OF OCCURRENCE:
On 2/20/17 two patients left the voluntary substance program against medical advice. Female patient (A) was in her 20s and male patient (B) was in his 20s.
The facility conducted an internal investigation and notified the physician and police. These two patients were voluntarily admitted to the substance abuse program. On this day, they expressed a desire to leave the program. Staff reported educating the patients on the consequences of their actions if they left the program. Staff recommended the patients see the mental health provider for an assessment prior to leaving the program. Staff notified the mental health provider about the situation, and prior to an assessment, both patients left the facility without their belongings. Staff called resident (A)'s probation officer about her leaving the voluntary program. The provider placed both patients on AWA status (Absent without Authority). Both patients had a diagnosis of mental illness with a history of substance abuse. Later, patient (A) called the unit and told a patient she was doing fine and living at a former patient's house. Patient (A) did not return to the program. Patient (B) returned to the unit on 2/21/17 demanding his personal belongings. One staff member reported they did not observe any significant issues with patient (B). Both patients were their own responsible parties and would be at risk for returning to substance abuse.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
