DESCRIPTION OF OCCURRENCE:
On 2/18/17 male patient (A), in his 30s, physically assaulted male patient (B), who was in his 20s.
The facility conducted an internal investigation and notified the police. It was reported patient (A) shoved past a security officer and entered the day hall. Patient (A) approached patient (B) and began punching him on the face. Patient (B) began punching back. Staff was able to intervene and separate the patients. Patient (A) was contained and escorted to the seclusion room. Nursing assessed patient (B) and noted bilateral periorbital edema/hematomas (bruising and swelling of the eye area). First aid treatment provided and Tylenol administered for pain. Staff would continue monitoring for any neurological changes. Nursing assessed patient (A) and provided first aid treatment to a cut on his hand. Both patients have a diagnosis of mental illness with behaviors. The patients resided on different parts of the hallway and assault II precautions were implemented for patient (A). In addition, new medications were started on patient (A) for behavioral management.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
