DESCRIPTION OF OCCURRENCE:
On 02/09/17 a female patient, in her 40s, eloped from the facility.
The facility conducted an internal investigation and notified the police department. The patient stated she was going to leave the Dual Diagnosis Chemical Dependency program. She refused to wait for a psychiatric evaluation or give an explanation as to why she wanted to leave the program. The patient attempted to obtain her property from storage, but was not able to so she returned to the unit. Staff met with the patient and she agreed to continue with the program. Later that day, other patients witnessed the patient leaving the facility grounds instead of going to the dining room. A search was conducted, but the patient was not located. Completion of the program was a condition of her probation. The patient called the facility the following day. She reported she met with her Probation Officer who instructed her to return to the program. She was informed that she would have to reapply to the program and could not just return. The program was a voluntary substance abuse treatment program and the patient was there on a voluntary status at the time that she left the facility. The patients Parole Officer was notified of her discharge against medical advice.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. However, the facility will be cited due to late reporting. The Department will review this occurrence prior to the next survey.
