DESCRIPTION OF OCCURRENCE:
On 02/12/17 male patient (A), in his 20s, hit male patient (B) who was in his 30s.
The facility conducted an internal investigation and notified the physician and police. The patients were arguing when staff approached them and attempted to redirect them. Patient (A) then hit patient (B) in the face. Staff were able to separate them. Patient (A) continued to be angry and resistive and fell to the floor while staff applied restraints. Several staff were injured during the altercation. Patient (A) was taken to the seclusion room and placed in locked door seclusion. Patient (B) was assessed and had several small scratches on his neck and a small cut to his nose. Patient (B) refused any treatment. Patient (A) was on assault precautions at the time of the incident. He was medicated for agitation.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings were based on the information provided by the facility at the time of the Department review.
