DESCRIPTION OF OCCURRENCE:
On 1/28/17, two patients eloped from the facility. Male patient (A) was in his 30s and female patient (B) was in her 20s.
The facility conducted an internal investigation and notified the physician, probation officer and police. One peer alerted staff that patient (A) and patient (B) left the program and the facility. Staff searched the unit and patient rooms. Although several personal items remained in the rooms, the bulk of their personal belongings had been removed. These two patients did not alert staff of their choice to leave the program. The patients were located out in the community not far from the facility grounds and they refused to return. Staff obtained a physician's order to place them on an absence without authority (AWA) status and then discharge orders were written. The patients had been admitted to the substance program under a voluntary status as part of their probationary conditions. They were both identified to be their own responsible parties with a diagnosis of mental illness and history of substance abuse. The facility felt the patients were not at risk to self or others; except for a possible return to drug use. Staff notified the patient's probation officer about their elopement from the program.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
