DESCRIPTION OF OCCURRENCE:
On 1/21/17 male patient (B), in his 50s, struck another male patient (A), who was in his 20s.
The facility conducted an internal investigation and notified the police and physician. It was reported patient (A) was in an agitated state and hit a security officer and another staff member with subsequent injuries. As additional staff continued to attempt containing patient (A), patient (B) struck patient (A) in the face twice. After being struck, patient (A) voluntarily walked to seclusion and staff directed patient (B) to his room. Nursing assessed patient (A) and he sustained a small abrasion to his right cheek and his mouth was bleeding. He complained of pain and first aid treatment was provided. Staff initiated medical monitoring on patient (A) for the next 24 hours. Both patients had a diagnosis of mental illness with a history of substance abuse and physical aggression. Patient (B) reported he hit patient (A) because of the assault to staff, but staff believed patient (B)'s action was retaliatory for a prior assault. Neither patient reported being fearful of one another. Assault II precautions and a peer restriction was started for both patients. Patient (B) was regressed to a higher security level unit the following day.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
