|Facility: COLORADO MENTAL HEALTH INSTITUTE AT PUEBLO-PSYCH
Date of Occurrence: 1/20/2017
Report Timely: Yes
Type of Occurrence: Misappropriation of Property
DESCRIPTION OF OCCURRENCE:
On 1/20/17 a male patient, in his 50s, reported several items were missing from the storage area inside the facility.
The facility conducted an internal investigation and notified the police and physician. While searching through the patient's property, he reported a few items were missing. The patient named books, legal work, photo album with photos, sleep masks and other papers were missing from storage. Review of the patient's inventory form, dated back to 2014, revealed he did have a box of books and a box of various paperwork at the time of admission. Staff searched the short and long-term storage areas and the items were not found. Some staff recalled a friend of the patient took a box containing books, sleep masks and paperwork out of the facility per the patient's request. The staff member was not able to clearly identify what exact books or papers were taken. No other patients reported a concern about missing items. Through the investigation, it was discovered staff did not follow proper procedures when the friend took items from the facility. Staff should have checked the specific items on the property log to update the information. The appointing authority reviewed the personal property policy with staff.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
