DESCRIPTION OF OCCURRENCE:
On 12/29/16 female patient (A), in her 40s, alleged she was verbally threatened by another female patient (B), who was in her 20s.
The facility conducted an internal investigation and notified the police. Patient (A) reported patient (B) was spreading rumors about her saying she was a baby killer. Patient (A) stated she was afraid for her safety because people get hurt in prison for that sort of behavior. Patient (A) also stated patient (B) had told her she was here for murder and she would murder patient (A). Patient (A) told staff she was scared. Both patients had a diagnosis of mental illness with behaviors. Staff have observed these two patients engage in verbal disagreements and patient (B) had a history of engaging in this sort of behavior. Both patients were put on a peer restriction from each other and are housed on separate wings of the unit. Staff also implemented extra wing observations for patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.
The Department will review this occurrence prior to the next survey.
