DESCRIPTION OF OCCURRENCE:
On 12/20/16 male patient (A), in his 30s, allegedly assaulted male patient (B), who was in his 20s.

FACILITY/AGENCY ACTION:
The facility conducted an internal investigation and notified the police. Staff observed patient (B) pinned against the wall and patient (A) was punching him repeatedly in the face. Staff immediately intervened and separated the patients. Patient (A) voluntarily left the area and walked to his room. Nursing assessed patient (B) and noted he had minor cuts to his lip with swelling and a reddened area to his left upper forehead. First aid treatment was provided and patient (B) returned to the facility with orders for staff to continue monitoring his condition. Patient (B) stated he was fearful of patient (A). Assault II precautions were implemented for patient (A) and his care plan was updated to include dangerous behaviors. Patient (B) reported he was attempting to give patient (A) his breakfast tray and patient (A) believed patient (B) had eaten the food on the tray. Patient (B) was moved to a different unit.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the department at the time of review.

The Department will review this occurrence prior to the next survey.
