DESCRIPTION OF OCCURRENCE:
On 11/27/16 male patient (A), in his 50s, and male patient (B), in his 60s, were involved in a physical altercation.
The facility conducted an internal investigation and notified the police and physician. Patient (A)'s current care plan addressed his legal, medical, and behavioral issues, including dangerousness and substance abuse. Patient (B) had a history of bipolar mood and delusional disorders. His current care plan addressed his legal, behavioral, medical, dangerousness, and substance abuse issues. On 11/27/16, patient (B) was upset about a televised football game and began to argue with patient (A). Patient (A) called patient (B) names and, in response, patient (B) hit patient (A) in the face approximately three times. Staff intervened and separated the patients. Patient (A) was assessed by a registered nurse with noticeable swelling and bruising to his left eye. The area was cleansed and an ice pack was applied. Patient (A) also had a small abrasion on his chest, which was cleansed. A medical provider assessed patient (A) and ordered x-rays. Patient (A)'s vital signs, x-rays, and neurological assessments were within normal limits. Patient (A) denied pain. Patient (B) was placed on specific assault precautions, peer restriction, and one-to-one line of sight monitoring. Patient (B)'s medications were also reviewed. Patient (A) had no ongoing concerns regarding the incident and patient (B) expressed remorse regarding his actions. The allegation of physical abuse was substantiated based on peer and staff witness statements. As of the time of the report, there were no further incidents between patients (A) and (B). Staff continued to monitor the patients closely.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, and conducting an investigation.
The summary findings were based on facility information provided to the Department at the time of review. The Department will review this occurrence prior to the next survey.
