DESCRIPTION OF OCCURRENCE:
On 11/26/16 male patient (A) and male patient (B) were involved in an altercation. Both patients were in their teens.
The facility conducted an internal investigation and notified the police. The patients were in the day hall. Patient (B) was seated on a couch. Patient (A) walked over to him and began to hit him with a closed fist on his left cheek. Patient (B) curled up in a fetal position and covered his face and head with his arms. Patient (A) then struck patient (B) with his fists on top of patient (B)'s head. Staff separated them. Patient (A) was taken to the Intensive Evaluation and Treatment area. Patient (B) was assessed and had slight redness to his left cheek which he said was sore. Vital signs and neurological checks were ordered and were within normal limits. Patient (A) was assessed and had slight swelling to his left knuckles. An x-ray of his hand was ordered. He was placed on Assault II precautions. Patient (B) was moved to a room on a different level of the unit to separate him from patient (A). The staff attempted to provided Medication Counseling but patient (B) was not willing to participate.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings are based on the information provided by the facility at the time of the Department review.
