DESCRIPTION OF OCCURRENCE:
On 11/27/16 female patient (A), in her 30s, scratched the foot of female patient (B). Patient (B) was in her 50s.
The facility conducted an internal investigation and notified police. Patient (A) was agitated with patient (B) and lunged at her with clenched fists. Patient (A) verbally threatened patient (B). Patient (B) was seated at the time and put her feet up in self-defense. Patient (A) reached out to grab patient (B)'s leg and scratched the top of her foot drawing blood. Staff intervened and escorted patient (A) to the seclusion room. Once she met the criteria for release from seclusion, she was moved to a different room in a different wing away from patient (B). Patient (B) was assessed and her injury cleaned and a Band Aid applied. Patient (A) was placed on Assault II precautions. Patient (B) denied being fearful of patient (A).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings are based on the information provided by the facility at the time of the Department review.
