DESCRIPTION OF OCCURRENCE:
On 11/20/16 female patient (B), in her 20s, reported receiving frightening phone calls from male patient (A) also in his 20s.
The facility conducted an internal investigation and notified the police. The female resided in a different building from the male. The reported she had a past relationship with the male. She said he had assaulted her numerous times and she was very afraid of him. Patient (B) said she was so afraid of patient (A) that she would rather go to jail rather than completing her substance abuse program. Staff counseled her and assured her of her safety. Patient (B) chose to stay and complete her program. Hospital police arrived on the unit to interview patient (A) who stated that he and patient (B) were friends. The physician was notified and placed patient (A) on phone restrictions. Staff will monitor him.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey. The summary findings are based on the information provided by the facility at the time of the Department review.
