DESCRIPTION OF OCCURRENCE:
On 11/6/16 male patient (A), in his 30s, physically assaulted another male patient (B), who was in his 60s.
The facility conducted an internal investigation and notified the police and physician. It was reported the two patients began arguing in the day hall while standing in line for dinner. Patient (A) suddenly punched patient (B) on the left side of his face which caused him to fall backwards. Staff quickly intervened and separated the patients. Patient (A) took a voluntary time out in his room. Nursing assessed patient (B) and found several facial abrasions, a small bruise near his ear and a small bump on the back of his head. Neurological checks were within normal limits and staff provided first aid treatment. Medical monitoring continued for the next 24 hours for patient (B) and he reported he was not fearful of patient (A). Both patients had a diagnosis of mental illness. Patient (A) had a history of aggressive and demanding behaviors. Patient (A) allegedly hit patient (B) because patient (B) would not give him his snack. Assault II precautions were implemented on patient (A) and his privileges were placed on hold. The plan of care for patient (A) was updated to reflect the assault and staff would continue monitoring both patients.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
