DESCRIPTION OF OCCURRENCE:
On 11/6/16 a male patient, in his late teens, eloped.
The facility conducted an internal investigation and notified the medical provider, police, family/guardian. It was reported staff were unable to locate the patient for lunch. A search was conducted of the facility and immediate areas. The patient was not located. Review of the records indicated the patient was present on a voluntary status and no special precautions had been in place. Staff reported the patient was not identified to be at risk to others but may be at risk for using substances. When contacting the family, they reported he showed up at a facility in Denver and his drug screen was negative for any substances. He had a diagnosis of mental illness with a history of substance abuse. The patient was discharged from this program.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
