DESCRIPTION OF OCCURRENCE:
On 10/31/16 a male patient (A), in his 50s, allegedly assaulted another male patient (B), who was in his 60s.
The facility conducted an internal investigation and notified the police. As the patients were returning to the unit, staff observed patient (A) say something to patient (B). After this verbal exchange, patient (A) forcefully pushed patient (B), who fell backwards and struck his head on the floor. Patient (A) then stood over patient (B) with his arm pulled back; ready to strike. Staff intervened and separated the patients. Staff escorted patient (A) back to his room and administered a medication to help patient (A) with his agitation. Nursing assessed patient (B) and noted he had a hematoma to the back of his head. Patient (B) denied pain and staff applied ice to the hematoma area. Neurological checks were initiated to monitor patient (B) for any changes. Assault II precautions and a peer restriction were implemented for patient (A). Patient (A) stated patient (B) was yelling at him and calling him names, which made him mad. Patient (B) moved to another unit for safety reasons as staff believed he had been antagonizing his peers.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
