DESCRIPTION OF OCCURRENCE:
On 10/23/16 a male patient, in his 30s, allegedly assaulted another male patient (B), who was in his 50s.
The facility conducted an internal investigation and notified the police and physician. It was reported patient (A) had been pacing the hallway, approached patient (B) and punched him in the face twice. Patient (A) walked away and went back to his room. Nursing assessed patient (B) as he complained of neck pain. Tylenol was administered for pain. Both patients had a history of mental illness with aggressive behaviors. Patient (A) was moved to a different unit and placed on assault II precautions with a new individualized behavioral plan in place. In addition, patient (A)'s medications were being reviewed. Staff were unable to determine what triggered patient (A) to hit patient (B).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
