DESCRIPTION OF OCCURRENCE:
On 10/10/16 a verbal and physical altercation occurred between two male patients. Patient (B) was in his 40s and patient (A) was in his 30s.
The facility conducted an internal investigation and notified the police and physician. During a yard break, patient (A) and patient (B) started arguing which led into a physical altercation. Subsequently, patient (B) pushed patient (A) and he fell to the ground. A duress alarm was called and additional staff arrived to assist with separating the patients. It appeared patient (A) was the aggressor and he was escorted to the seclusion room to calm down. Nursing assessed patient (B) and staff noted he sustained a cut which was treated. Patient (A) sustained a cut on his lip and the area was cleansed. After seclusion, patient (A) was moved to another unit and assault II precautions implemented. In addition, patient (A)'s medications were adjusted. Patient (B) reported patient (A) started making rude comments about his mother and stated he asked patient (A) if he wanted to fight. Both patients had a history of mental illness with aggressive behaviors. No police charges were filed.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
