DESCRIPTION OF OCCURRENCE:
On 10/07/16 male patient (A), in his 30's, walked into male patient (B)'s room and hit patient (B) on the left side of the face. Patient (B) was in his 50's.
The facility conducted an internal investigation and notified the police and physician. Patient (A) had an extensive assault history. Staff observed patient (A) responding to internal stimuli (such as hearing voices) on a daily basis. Patient (B) had a history of mental illness. On 10/07/16, patient (A) walked into patient (B)'s room and hit him on the left side of the face. Patient (A) stated to patient (B), "Now go and snitch me out!" Patient (A) alleged that patient (B) was calling patient (A)'s family member a vulgar name. Patient (B) reported he was unaware of any arguments or other issues with patient (A) prior to the assault. Staff separated the patients and a nurse assessed patient (B). Patient (B) was provided a pain reliever for complaints of pain to his left jaw. Patient (B)'s vital signs and neurological checks were normal. Patient (A) was regressed to a different unit. Patient (B) stated he was not concerned for his safety once patient (A) was moved. The physician wrote orders to place patient (A) on assault precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.
The Department will review this occurrence prior to the next survey.
