DESCRIPTION OF OCCURRENCE:
On 09/30/16 male patient (A), in his 20's, assaulted male patient (B), who was also in his 20's.

FACILITY ACTION:
The facility conducted an internal investigation and notified the police and physician. Patient (A) had a history of behavioral issues and psychotic symptoms, including grandiose delusions and delusions of being controlled by spirits. Patient (B) had a history of mental health issues requiring multiple hospitalizations. On 09/30/16, patient (B) was sitting in the day hall reading the newspaper when patient (A) approached and hit him on the left side of his face. Staff who witnessed the event immediately intervened and separated the patients. Patient (A) was escorted to his room. Patient (B) was assessed with redness near his jaw, but no discernible swelling. Patient (B) reported he was experiencing a "popping feeling" when he opened his mouth. Patient (B) refused a pain reliever. The physician ordered an x-ray of the injured area, which was completed the same day. The x-ray was negative for fracture. Patient (B) identified he wanted to press charges against patient (A). Hospital police were notified of the request. Staff made arrangements to move patient (A) to a different wing. The physician wrote orders to place patient (A) on assault precautions. The facility planned to evaluate patient (A)'s medications. Patient (B) experienced no significant changes in his behavior and was soon back to his normal routine.

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, and conducting an investigation. The summary findings were based on facility information provided to the Department at the time of review.

The Department will review this occurrence prior to the next survey.
