DESCRIPTION OF OCCURRENCE:
On 09/25/16 a female patient (A), in her 50s, verbally threatened another female patient (B), in her 40s.
The facility conducted an internal investigation and notified the physician and the police department. Patient (A) began yelling at patient (B) stating she was going to kill her when she got out of the facility. The patients were familiar with each other as they had both been admitted from the County Jail on the same day. The police interviewed patient (A) and she denied making any threatening statements to patient (B). Patient (B) stated she felt threatened and was upset. Both patients had mental health diagnosis and a history of behavior problems which had been care planned. Staff witnessed the patients threatening each other and patient (A) was placed on assault II precautions and moved to another unit away from patient (B),
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
