DESCRIPTION OF OCCURRENCE:
On 09/03/16 a male patient (A), in his 30s, struck another male patient (B), also in his 30s, on the side of his head.
The facility conducted an internal investigation and notified the police department. Patient (A) walked up to patient (B), who was sitting on the couch, and struck him on the left side of his head. Patient (A) stated he assaulted patient (B) because he thought patient (B) had been in his room rummaging through his items. Patient (A) walked to his room for a voluntary time out. Both patients had a mental health diagnosis and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have slight redness to his left ear. Both patients were placed on peer restriction. Patient (A) was also placed on assault II precautions and his medications were adjusted.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
