DESCRIPTION OF OCCURRENCE:
On 08/27/16 male patient (A), in his 20s, attacked male patient (B) who was in his 30s. Patient (B) retaliated and hit patient (A) in the face.
The facility conducted an internal investigation and notified the police and physician. Patient (A) ran into patient (B)'s room and attacked him. Patient (B) retaliated and hit patient (A). Staff sounded the duress alarm and entered the room. Staff removed patient (A) and placed him in his room. The nurse assessed patient (B) who had a small scratch to his face and lip that was bleeding slightly. The area was cleansed and vital signs were assessed and were within normal results. Patient (B) denied having any other injuries. Patient (A) was assessed and had swelling to his left eye. He denied having any other injuries. The nurse provided patient (A) with an ice pack to apply to his left eye. The physician placed patient (A) on assault II precautions. Patient (A)'s medications were being evaluated.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
