DESCRIPTION OF OCCURRENCE:
On 08/09/16 male patient (A), in his 40's, alleged he was hit by male patient (B), who was in his 30's.
The facility conducted an internal investigation and notified the police. Patient (A) was diagnosed with delusional disorder (persecutory type). He was being ruled out for schizotypal or paranoid personality disorder. He was admitted to the facility for a competency evaluation related to charges of felony menacing, real/simulated weapon, and carrying a concealed weapon. He had an angry affect and had been observed advising peers to disregard unit rules and refuse medication. Patient (B) was diagnosed with schizophrenia (chronic, paranoid type), polysubstance abuse, and antisocial personality disorder. He had been placed on emergency medication due to aggressive/assaultive behavior. He was subsequently placed on court-ordered medication and his aggressive/assaultive behaviors decreased over time. Patients (A) and (B) were housed on different wings of the unit. On 08/09/16, patients (A) and (B) were playing dominos at a table in the facility's yard. Staff heard one of the patients yell expletives and saw both patients stand up. Patient (B) was encouraged to walk away with staff. Patient (A) retreated to the bathroom. When patient (A) exited the bathroom, he told staff he was hit by patient (B). Patient (A) was escorted to the wing of his unit and assessed by nursing staff. His left cheek was noted to be swollen. He refused vital signs but requested Tylenol and an ice pack, which were provided. He refused to rate his pain or to be assessed in the medical clinic. Patient (A) denied fearing patient (B). Patient (B) was placed on assault precautions. He denied hitting patient (A). Both patients were placed on wing observation. Facility police attempted to interview patient (A) but he refused to cooperate and became threatening toward police, nursing staff, and practitioners. Patient (A) indicated he planned to retaliate against patient (B) and was placed on assault precautions. Patient (A) was also started on an emergency medication as he was considered a danger to others.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
