DESCRIPTION OF OCCURRENCE:
On 07/26/16 a male patient (A), in his 60s, struck a female patient (B), in her 40s.
The facility conducted an internal investigation and notified the physician and the police department. Patient (B) was sitting by the phone in the hall when patient (A) walked over and punched patient (B) on the right side of her face. Staff took patient (A) and placed him in locked door seclusion. Patient (B) was assessed and noted to have slight pain on her lower right jaw area with slight redness noted. Both patients had a mental health diagnosis and a history of behavior problems which had been care planned. Patient (B) was interviewed and stated she did not know why patient (A) hit her. Patient (A) was not able to be interviewed as he continued to present with severe psychotic symptoms. Patient (A) was placed on one-to-one monitoring due to his unpredictable behaviors.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
