DESCRIPTION OF OCCURRENCE:
On 07/20/16 male patient (B), in his 20s, engaged in a physical altercation with another male patient (A), who was in his 40s.
The facility conducted an internal investigation and notified the police and physician. Staff responded to grunts coming from patient (A)'s room. Upon entering, staff observed patient (A) and patient (B) standing in a boxing stance with fists clenched and held up. Staff restrained patient (B) and removed patient (A) from the area. Patient (B) continued to struggle with staff and verbally commented he wanted to "knock the (derogatory)" out of someone. Staff proceeded to accompany patient (B) to the seclusion room. Upon assessment, patient (B) sustained scratches and bumps to his head. First aid was provided. Patient (A) did not sustain any injuries. Assault II precautions and a peer restriction was implemented on patient (B) because he entered patient (A)'s room and initiated the fight. Staff moved patient (A) to a different wing and staff planned to continue monitoring.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
