DESCRIPTION OF OCCURRENCE:
On 07/17/16 male patient (A) was struck by male patient (B). Both patients were in their 20s.
The facility conducted an internal investigation and notified the police and physicians. A staff member was in the day hall when she observed patient (B) approach patient (A) who was sitting on the couch, and begin hitting patient (A) numerous times in the face. The staff member was able to separate the patients, and had them return to their rooms that were located on separate wings. Patient (A) was assessed and had a small cut to his lower lip that was bleeding slightly. No sutures were needed. The area was cleansed and pressure applied. Neurological checks were performed, and resident (A) was given a Tylenol and Haldol as needed. Resident (B) was assessed on the unit by his physician who wrote orders to place resident (B) on assault II precautions. Resident (B) had a small scrape to the middle knuckle that was cleansed with soap and water. Staff member reported that it was an unprovoked assault. Staff continued to monitor both patients closely.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The facility failed to report timely and deficient practice was cited. The Department will review this occurrence prior to the next survey.
