DESCRIPTION OF OCCURRENCE:
On 7/8/16 two male patients got into an altercation. Both patients were in their 20s.
The facility conducted an internal investigation and notified the police. The beginning of the altercation was not witnessed. Staff observed the patients fighting in the courtyard. They were on the ground and patient (B) was on the bottom, but they were both aggressively throwing punches. Staff yelled for them to stop and placed both patients in a hold. Both patients followed staff direction and were escorted to locked door seclusion. Patient (A) sustained a 1/4 inch abrasion across three knuckles of his right hand. The wound was cleaned and antibiotic ointment applied. Patient (B) sustained abrasions on his right elbow and right knee. The wounds were cleaned and antibiotic ointment applied. Both patients were psychiatrically unstable and did not offer an explanation for the fight. Patient (A) stated he was fearful of patient (B). Mediation was performed and the patients came to a verbal resolution that allowed patient (A) to feel safe. Both of the patients reported that they had started the fight. Both of the patients were placed on assault II precautions, and both were placed on a wing observation.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
