DESCRIPTION OF OCCURRENCE:
On 06/26/16 two male patients engaged in a physical altercation. Patient (A) was in his 50s and patient (B) was in his 20s.
The facility conducted an internal investigation and notified the police and physician. Patient (A) and patient (B) engaged in a physical altercation of grabbing each other and then punching each other. Staff intervened and the patients separated from each other. Staff reported patient (B) remained agitated and took a voluntary time out in the seclusion room. Both residents sustained injuries from the altercation and first aid treatment was provided. Both patients reported the other started the altercation and assault II precautions were implemented. In addition, a peer restriction was implemented and patient (B) moved to another wing of the unit. Neither patient reported being fearful of one another.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons/agencies and conducting an investigation.
The Department will review this occurrence prior to the next survey.
